Synopsis:

Requiring specified insurers, nonprofit health service plans, and health maintenance organizations to provide specified coverage and payment for prosthetic devices; authorizing specified insurers, nonprofit health service plans, and health maintenance organizations to require prior authorization for coverage of a prosthetic device; providing that specified covered benefits may be subject to a specified copayment or coinsurance amount; etc.
