Health Care Decision Making Forms - Health Insurance Portability and Accountability Act - Personal Representatives


Synopsis:

Clarifying that a health care agent is a personal representative in order to receive specified health care information in accordance with the Health Insurance Portability and Accountability Act; and clarifying that a health care agent is a personal representative regarding an individual's capacity to make an informed decision in accordance with the Health Insurance Portability and Accountability Act.
